В октябре 2024 года в России зарегистрирован новый антибиотик — пазуфлоксацин. В отечественной литературе информация по эффективности и безопасности применения инъекционного пазуфлоксацина отсутствует.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Выполнить систематический обзор литературы и метаанализ данных по эффективности и безопасности применения лекарственного препарата (ЛП) пазуфлоксацин, раствор для инфузий.</p></sec><sec><title>Методы</title><p>Методы. В настоящий обзор были отобраны клинические исследования (КИ), в которых оценивалась эффективность и безопасность инъекционного пазуфлоксацина, выполненные на взрослых добровольцах с учётом нозологий, при которых ЛП показан к применению в РФ. Метаанализ данных проведён на основании опубликованных результатов рандомизированных клинических исследований (РКИ), выполненных в группе взрослых пациентов с инфекциями мочевыводящих путей (ИМП).</p></sec><sec><title>Результаты</title><p>Результаты. Из 1068 найденных статей, в настоящий обзор было включено 10 публикаций. Четыре исследования имели дизайн — РКИ, а оставшиеся относились к исследованиям «до и после». Эффективность пазуфлоксацина варьировала в зависимости от локализации, формы инфекции и дозы препарата, и составляла: от 78,7 до 100 % при ИМП, от 75,1 до 100 % при инфекциях дыхательных путей и от 80,0 до 95,2 % при лечении акушерско-гинекологических инфекций. Не смотря на наличие тенденции к более высоким значениям показателей эффективности при использовании пазуфлоксацина в РКИ, синтез данных, выполненный в рамках метаанализа, подтвердил отсутствие статистически значимых различий между сравниваемыми группами по показателям эффективности. Стоит отметить, что с момента проведения большинства исследований прошло 15 лет и более. Учитывая увеличение резистентности возбудителей инфекций к антибиотикам с течением времени, эффективность препаратов в российской популяции может отличаться от таковых в РКИ. Это в первую очередь относится к ЛП сравнения, имеющим историю их длительного применения на территории России. В отличие от ЛП сравнения, пазуфлоксацин является новым препаратом для российской клинической практики, а, следовательно, имеет более низкие риски антибиотикорезистентности. В большинстве КИ пазуфлоксацин хорошо переносился пациентами.</p></sec><sec><title>Выводы</title><p>Выводы. Согласно данным КИ, пазуфлоксацин является высокоэффективным при применении в рамках зарегистрированных в РФ показаний, а также обладает высоким профилем безопасности. Использование антибактериальных ЛП должно учитывать актуальные данные по резистентности возбудителей инфекций к назначаемым ЛП.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. In October 2024, a new antibiotic, pazufloxacin, was registered in Russia. Information on the efficacy and safety of injectable pazufloxacin is lacking in the Russian literature.</p></sec><sec><title>Objective</title><p>Objective. To conduct a systematic literature review and meta-analysis of data on the efficacy and safety of pazufloxacin, an infusion solution.</p></sec><sec><title>Methods</title><p>Methods. This review included clinical trials (CTs) evaluating the efficacy and safety of injectable pazufloxacin, conducted in adult volunteers, considering the clinical conditions for which the drug is indicated in the Russian Federation. A meta-analysis was performed based on published results of randomized clinical trials (RCTs) conducted in adult patients with urinary tract infections (UTIs).</p></sec><sec><title>Results</title><p>Results. Of the 1,068 articles identified, 10 were included in this review. Four studies were RCTs, and the remaining studies were before-and-after studies. The efficacy of pazufloxacin varied depending on the site, type of infection, and dose, ranging from 78.7 % to 100 % for UTIs, 75.1 % to 100 % for respiratory tract infections, and 80.0 % to 95.2 % for obstetric and gynecological infections. Despite a trend toward higher efficacy values for pazufloxacin in RCTs, a meta-analysis confirmed the absence of significant differences in efficacy between the compared groups. Notably, 15 years or more have elapsed since most studies were conducted. Given the increasing antibiotic resistance of infectious agents over time, the efficacy of drugs in the Russian population may differ from that in RCTs. This primarily applies to the comparator drugs considering a history of their long-term use in Russia. Unlike the comparator drugs, pazufloxacin is a new drug with a lower risk of antibiotic resistance in Russian clinical practice. In most clinical trials, pazufloxacin was well tolerated by patients.</p></sec><sec><title>Conclusions</title><p>Conclusions. According to CTs in the Russian Federation, pazufloxacin has a high safety profile and is highly effective when used within its approved indications.
