Клинические проявления гипопитуитаризма разнообразны, часто недостаточность тропных гормонов не имеет ярко выраженной симптоматики, однако нарушение функции гипофиза зависит от степени тяжести дефицита гормонов. Наиболее распространёнными причинами первичного гипопитуитаризма являются аденома гипофиза и осложнения после хирургического вмешательства или лучевой терапии для лечения аденомы гипофиза. Помимо наличия образований гипофиза, различные повреждения головного мозга, такие как черепно-мозговая травма, ятрогенная травма во время операции или краниальное облучение, также могут вызывать гипопитуитаризм. Гипопитуитаризм, возникший вследствие радиационного облучения головного мозга, возникает очень редко, однако в литературе описываются случаи с ведением пациентов, получивших курс лучевой терапии. Несмотря на актуальность данной проблемы, механизм развития патологии недостаточно хорошо изучен. Пациенты, столкнувшиеся с ятрогенным гипопитуитаризмом, в результате длительного динамического наблюдения позволили проследить сходство в последовательном развитии симптоматики, при этом каждый случай отличается уникальностью клинической картины.</p></abstract><trans-abstract xml:lang="en"><p>Hypopituitarianism is a chronic disease based on insufficient secretion of hormones in the anterior pituitary gland. Clinical manifestations of hypopituitarism are varied, often the lack of tropic hormones does not have pronounced symptoms, but the impairment of the function of the pituitary depends on the severity of the hormone deficiency. The most common causes of primary pituitarism are pituitary adenoma and complications after surgery or radiation therapy to treat pituitary adenoma. In addition to the formation of a pituitary gland, various types of brain injury such as craniocerebral trauma, iatrogenic trauma during surgery or cranial radiation can also cause a pituitary syndrome. Hypopituitarism caused by radiation exposure to the brain is very rare, but cases have been reported in the literature of patients undergoing radiation therapy. Despite the relevance of this problem, the mechanism of development of pathology is not well researched.
