Сама РСВ-инфекция в большинстве случаев протекает в лёгкой или среднетяжёлых формах, не требующих госпитализации. Преобладание нетяжёлых форм РСВ-инфекции у детей не означает отсутствие последствий заболевания для экономики страны и общества в целом. При тяжёлых формах заболевания может потребоваться госпитализация, что существенно увеличивает нагрузку на бюджет.</p></sec><sec><title>Цель </title><p>Цель . Оценка социально-экономического бремени РСВ-инфекции у детей первых двух лет жизни в Российской Федерации на основе комбинации реальных данных эпидемиологического надзора, экспертного распределения пациентов по тяжести течения заболевания и моделирования затрат.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В ретроспективное исследование для моделирования бремени за 1 год (один сезон) включены данные обезличенной базы традиционного лабораторного надзора за гриппом и ОРВИ ФГБУ «НИИ гриппа им. А. А. Смородинцева» в сезоны 2022–2023, 2023–2024 и 2024–2025 гг. На основании подтвержденных лабораторно показателей заболеваемости РСВИ в городах России, находящихся под непрерывным многолетним мониторингом (минимально 5,8 % максимально 12,3 % в структуре ОРВИ), проведена экстраполяция на всех детей 0–2-летнего возраста в стране. Минимальное расчётное число случаев РСВИ в сезон 245 723, максимальное 521 101. Эксперты выделили 4 модели пациентов на основании тяжести течения, необходимости в госпитализации, в том числе в отделение реанимации, осложнений заболевания. Проведена оценка прямых медицинских (скорая помощь, стационарное и амбулаторное лечение, лекарственная терапия), прямых немедицинских (выплаты по временной нетрудоспособности родителю по уходу за больным ребёнком) и непрямых затрат (недополученный ВВП в связи с уходом родителя за ребёнком, а также стоимость потерянных лет жизни при летальном исходе у ребёнка). Горизонт анализа один эпидемический сезон с учётом отдалённых экономических последствий при смертельном исходе в процессе госпитализации (дисконтирование 3,5 %).</p></sec><sec><title>Результаты</title><p>Результаты. Суммарные прямые медицинские затраты на лечение в условиях стационара могут достигать 12,9 млрд. руб., а в амбулаторных условиях 1,01 млрд. руб. в год (сезон) в результате РСВ-инфекции у детей 0–2 летнего возраста. Прямые немедицинские затраты рассчитаны как 9,6 млрд руб., а недополученный вследствие ухода за ребёнком ВВП не менее, чем 7,7 млрд. руб. Итого общие затраты оценены в сумму более 32 млрд. руб. При определении социально-экономического бремени РСВИ с учётом расходов на один сезон и стоимости непрожитых лет жизни общие потери как бюджета, так и общества могут составить 3,31 трлн. руб. (минимально) или 7,01 трлн. руб. (максимально). Анализ чувствительности подтвердил устойчивость модели к изменению параметра заболеваемости.</p></sec><sec><title>Заключение</title><p>Заключение. Социально-экономическое бремя РСВ-инфекции у детей 0–2 лет достигает существенных размеров и характеризуется прогрессивным ростом при увеличении заболеваемости. Расходы здравоохранения, потери в экономике и социальной сфере диктуют необходимость совершенствования мер профилактики у наиболее уязвимой возрастной группы детей 0–2 лет жизни.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Actuality</title><p>Actuality. Respiratory syncytial viral (RSV) is a most common reason for acute respiratory infections in infants and children. Prevalence of non-severe RSV infection forms in kids does not mean absent it's influence for country economics and society at all. Severe cases of RSV infections could require hospitalization, that increases budget impact.</p></sec><sec><title>Objective</title><p>Objective. Social-economic burden of RSV infection evaluation in children first two years of life in Russian Federation basing on real data of epidemiologic monitoring, experts’ opinion for patients’ separation in depends on severity of illness and modelling of expenditures.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. Data of non-personal base of traditional flu and acute respiratory infections laboratory monitoring of National institute of flu named after Smorodintsev (seasons 2022–2023, 2023–2024, 2024–2025 yy) were included into retrospective analysis for burden modelling for 1 year (season). Extrapolation of monitoring results of proven laboratory indicators of RSV infection prevalence in cities of detection (minimum 5,8 %, maximum 12,3 % out of prevalence of acute respiratory viral infections), were made on all kids 0–2 y. o. population in the country. Minimal calculated amount of RSV infection was 245 723, maximal 521 101 per season. Four models of patients based on illness severity and necessity in hospitalization, including ICU, complications have been defined by medical experts. Direct medical (ambulance, outand in-patient treatment) indirect medical (cost of parental seek live) as well as indirect costs (GDP loss in case of parental seek live and cost of life years loss due to premature death of child) were calculated. Horizon of analysis one epidemic season, and long-term economic consequences in case of death during hospitalization (discount rate 3.5 %).</p></sec><sec><title>Results</title><p>Results. Total direct medical expenditures in hospitals can reach 12.9 bln RUR, and for out-patient treatment 1.01 bln RUR annually (season) of RSV infection. Direct non-medical expenditures were calculated as 9.6 bln RUR, and GDP loss due to parental seek live no less than 7.7 bln RUR. Total common expenditures were reached in 32 bln RUR. If we can calculate burden of RSV infection in case of premature death in severe illness total budget loss could reach 3.31 trillion RUR (minimal) or 7.01 trillion RUR (maximal). Sensitivity analysis confirmed stability of model to prevalence RSV infection parameter changing.</p></sec><sec><title>Conclusion</title><p>Conclusion. Social-economic burden of RSV infection in children 0–2 y. o. has a huge size and has progressive dynamic in case of prevalence increasing.
