Инфаркт миокарда (ИМ) остаётся одной из ведущих причин смертности и инвалидности во всём мире. Высокая распространённость этого атеросклерозассоциированного заболевания связана с недостаточной профилактикой сердечно-сосудистых заболеваний, в том числе вследствие недостаточного применения статинов. В настоящей статье освещена проблема оценки применения статинов как средства первичной и вторичной сердечно-сосудистой профилактики на примере инфаркта миокарда из выборки пациентов сибирского региона.</p></sec><sec><title>Цель</title><p>Цель. Провести оценку частоты применения статинов у больных до наступления первичного и повторного инфаркта миокарда 1‑го типа.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В открытое сплошное поперечное исследование было включено 294 больных, отобранных из локального регистра острого коронарного синдрома, пролеченных на базе 1 кардиологического отделения ГБУЗ ГКБ № 1 Новосибирска в 2021–2023 гг. Для оценки влияния различных факторов на применение статинов на догоспитальном этапе была использована модель многофакторной логистической регрессии.</p></sec><sec><title>Результаты</title><p>Результаты. Установлено, что в подгруппе первичного ИМ частота приёма статинов составляла 21,8 %, а в подгруппе повторного ИМ — 38,6 %. При анализе факторов, ассоциированных с фактом применения статинов, выявлена статистически значимая связь с повторным инфарктом миокарда, отношение шансов (ОШ) 2,26 (1,29–3,99), и с дислипидемией, ОШ 1,81 (1,02–3,25). Другие факторы: артериальная гипертония, сахарный диабет 2 типа, ожирение, фибрилляция предсердий, фракция выброса левого желудочка, уровень креатинина, возраст и пол не достигли уровня значимости. Дозы препаратов соответствовали клиническим рекомендациям; в 70 % случаев применялся аторвастатин, в 30 % — розувастатин. Среди принимавших статины пациентов многие (более 2/3 пациентов) не достигали целевых значений холестерина липопротеинов низкой плотности.</p></sec><sec><title>Выводы</title><p>Выводы. Настоящее исследование демонстрирует недостаточную частоту, интенсивность и продолжительность терапии статинами среди пациентов, госпитализирующихся с первичным и повторным инфарктом миокарда 1‑го типа, и это свидетельствует о плохом состоянии первичной и вторичной профилактики данного предотвратимого сердечно-сосудистого события.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. Myocardial infarction (MI) remains one of the leading causes of death and disability worldwide. The high prevalence of this atherosclerosis-associated disease is undoubtedly associated with insufficient prevention of cardiovascular diseases, including due to insufficient use of statins. This article highlights the problem of evaluating the use of statins as a means of primary and secondary cardiovascular prevention using the example of myocardial infarction from a sample of patients in the Siberian region.</p></sec><sec><title>Objective</title><p>Objective. To evaluate the frequency of statin use in patients before the onset of primary and recurrent type 1 myocardial infarction.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The open continuous cross-sectional study included 294 patients selected from the local registry of acute coronary syndrome, treated at the 1st cardiology department of the Novosibirsk State Clinical Hospital No. 1 in 2021–2023. A multifactorial logistic regression model was used to assess the effect of various factors on the use of statins at the prehospital stage.</p></sec><sec><title>Results</title><p>Results. It was found that in the subgroup of primary MI, the frequency of statin use was 21.8 %, and in the subgroup of repeated MI, the frequency was 38.6 %. An analysis of the factors associated with statin use revealed a statistically significant association with repeated myocardial infarction, OR 2.26 (1.29–3.99), and with dyslipidemia, OR 1.81 (1.02–3.25). Other factors: hypertension, diabetes mellitus type 2, obesity, atrial fibrillation, left ventricular ejection fraction, creatinine levels, age and gender. They have not reached the level of significance. The doses of the drugs corresponded to clinical recommendations; in 70 % of cases, atorvastatin was used, and in 30 %, rosuvastatin was used. Among the patients taking statins, many (more than 2/3 of the patients) did not reach the target values of LDL cholesterol.</p></sec><sec><title>Conclusions</title><p>Conclusions.
