Иммуновоспалительные ревматические заболевания (ИВРЗ) представляют собой значительную медико-социальную проблему, ведущую к высокой смертности, инвалидизации и экономическому бремени для общества. Эти заболевания, такие как ревматоидный артрит (РА), псориатический артрит (ПсА) и аксиальный спондилоартрит (аксСпА), существенно снижают качество жизни пациентов и увеличивают финансовую нагрузку на системы здравоохранения.</p></sec><sec><title>Цель</title><p>Цель. Цель настоящего систематического обзора — оценить текущие данные о риске смерти у пациентов с ИВРЗ и выявить факторы, ассоциированные с повышенной летальностью, а также проанализировать эффективность биологических препаратов в снижении этого риска.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Обзор включает данные из множества клинических исследований и метаанализов, охватывающих период с 2000 по 2021 год. Анализ проводился на основе данных, собранных из различных источников, включая национальные регистры, клинические исследования и метаанализы.</p></sec><sec><title>Результаты</title><p>Результаты. Основные результаты показывают, что ИВРЗ, такие как РА, ПсА и аксСпА, значительно увеличивают риск смерти по сравнению с общей популяцией (SMR 2,02 1,79; 2,29). Активность заболевания, курение и коморбидные состояния являются ключевыми факторами, увеличивающими риск летальности. Использование генно-инженерных биологических препаратов (ГИБП) в терапии снижает риск смерти по сравнению с отсутствием биологической терапии (ОР (HR) 0,52 0,32; 0,86) и по сравнению с метотрексатом (ОР (HR) 0,77 0,60; 0,97). Применение ГИБП также снижает риск сердечно-сосудистых событий и развития деменции.</p></sec><sec><title>Выводы</title><p>Выводы. Раннее и активное использование ГИБП может стать значимым фактором в снижении негативных последствий ИВРЗ, включая смертность и прогрессирование сердечно-сосудистых заболеваний.</p></sec><sec><title>Заключение</title><p>Заключение. Результаты систематического обзора могут быть полезны для разработки стратегий управления ИВРЗ и оптимизации медицинской помощи пациентам. Внедрение ГИБП в терапию ИВРЗ может существенно улучшить прогноз и качество жизни пациентов, снижая риск летальности и других неблагоприятных исходов.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>   Background</title><p>   Background. Immunoinflammatory rheumatic diseases (IIRDs) are a significant medical and social problem leading to high mortality, disability and economic burden for society. These diseases, such as rheumatoid arthritis (RA), psoriatic arthritis (PsA) and axial spondylitis (axSpA), significantly reduce the quality of life of patients and increase the financial burden on healthcare systems.</p></sec><sec><title>   Objective</title><p>   Objective. The aim of this systematic review is to assess current data on the risk of death in patients with IIRDs and identify factors associated with increased mortality, as well as analyze the effectiveness of biological drugs in reducing this risk.</p></sec><sec><title>   Materials and methods</title><p>   Materials and methods. The review includes data from multiple clinical trials and meta-analyses covering the period from 2000 to 2021. The analysis was based on data collected from various sources, including national registries, clinical trials and meta-analyses.</p></sec><sec><title>   Results</title><p>   Results. The main results show that IIRDs such as RA, PsA and axSpA significantly increase the risk of death compared to the general population (SMR 2.02 1.79; 2.29). Disease activity, smoking and comorbid conditions are key factors increasing the risk of mortality. The use of biological agents in therapy reduces the risk of death compared to no biological therapy(HR 0.52 0.32; 0.86) and compared to methotrexate (HR 0.77 0.60; 0.97). The use of biological agents also reduces the risk of cardiovascular events and the development of dementia.</p></sec><sec><title>   Conclusions</title><p>   Conclusions. Early and active use of biological agents can be a significant factor in reducing the negative consequences of IRDs, including mortality and the progression of cardiovascular diseases.</p></sec><sec><title>   Conclusion</title><p>   Conclusion. The results of the systematic review may be useful for developing strategies for managing IIRDs and optimizing medical care for patients.
