Abstract
The Resolution of the Congress of Clinical Pharmacologists, held on May 21–22, 2026 in Moscow within the framework of the IV Russian Congress "Pharmacotherapy Safety 360°: Noli nocere!", defines the priority objectives of the professional community for the period up to 2030. The document specifies the pathways for implementing the Healthcare Development Strategy of the Russian Federation (Presidential Decree No. 896 of 08.12.2025) by strengthening the role of clinical pharmacologists in ensuring effective, safe, and cost-effective pharmacotherapy, introducing domestic medicines, pharmacovigilance, curbing antimicrobial resistance, personalized medicine, and combating polypharmacy. The Resolution formulates 19 specific items with designated responsible parties and deadlines, covering the development of a specialty strategy, updating the regulatory framework, integrating clinical pharmacologist consultations into the clinical-statistical groups system, participation in working groups on clinical guidelines, establishing a national research center, advancing educational programs (undergraduate, residency, continuing professional education), organizing formulary committees, as well as activities for career guidance, accreditation, and enhancing the prestige of the specialty. Special emphasis is placed on integrating clinical pharmacological technologies (pharmacogenetic testing, therapeutic drug monitoring, deprescribing) into clinical guidelines and preparing for the 30th anniversary of the specialty in 2027.

The participants of the Congress consider the following as the top‑priority task of the professional community of clinical pharmacologists:
Clinical pharmacologists play a key role in the implementation of the Healthcare Development Strategy until 2030 (Presidential Decree of the Russian Federation No. 896 of December 8, 2025 "On the Healthcare Development Strategy of the Russian Federation for the Period up to 2030") in the following areas, which will contribute to increasing life expectancy and improving the quality of life of the population:
ensuring effective, safe and cost‑effective pharmacotherapy for patients;
building technological sovereignty and introducing domestic medicinal products into clinical practice;
pharmacovigilance in medical organisations and surveillance of adverse events after immunisation;
curbing antimicrobial resistance;
implementing personalised medicine technologies, including pharmacogenetics and therapeutic drug monitoring;
combating polypharmacy and inappropriate prescribing, improving patient adherence to pharmacotherapy and promoting responsible self‑medication.
To achieve this objective, it is necessary to:
1. Develop and agree with the Ministry of Health of Russia a Strategy for the Development of the "medical" specialty "Clinical Pharmacology" until 2030.
Executors: jointly the Chief Freelance Specialist (CFS), the Profile Commission, the Eurasian Association of Clinical Pharmacology and Pharmacovigilance, and the Association of Clinical Pharmacologists.
Deadline: October 1, 2026.
2. Formulate proposals to increase the demand for clinical pharmacologists in medical organisations, as well as their role as experts at the regional and federal levels (expert work on issues of interchangeability and pharmacotherapeutic substitutions, "embedding" the consultation of a clinical pharmacologist into clinical guidelines, quality criteria for medical care, standards of medical care, etc.).
Executors: jointly the CFS, the Profile Commission, the Eurasian Association of Clinical Pharmacology and Pharmacovigilance, and the Association of Clinical Pharmacologists.
Deadline: November 1, 2026.
3. Update the regulatory legal acts concerning the activities of clinical pharmacologists and drug provision:
the procedure for providing medical care in the profile "Clinical Pharmacology";
Order of the Ministry of Health No. 494 "On Improving the Activities of Clinical Pharmacologists";
the professional standard for a clinical pharmacologist;
Order of the Ministry of Health No. 1094n "On Approval of the Procedure for Prescribing Medicinal Products, Forms of Prescription Forms for Medicinal Products, the Procedure for Completing These Forms, Their Recording and Storage, Forms of Prescription Forms Containing Prescriptions for Narcotic Drugs or Psychotropic Substances, the Procedure for Their Manufacture, Distribution, Registration, Recording and Storage, as well as the Rules for Completing Prescription Forms, including in the Form of Electronic Documents".

4. Formulate proposals to include a consultation by a clinical pharmacologist in the clinical‑statistical groups (CSG) system (when submitting a patient's case for payment (with any ICD code) if the medical record contains a consultation by a clinical pharmacologist). Submit a request to the Central Research Institute for Healthcare Organisation and Informatisation (TsNIOIZ) and the Ministry of Health of Russia to obtain the results of the time‑motion study of a clinical pharmacologist (2023) for the subsequent submission of a request to the Ministry of Health of Russia to develop a regulatory act defining standard workload norms for a specialist physician.
Executors: jointly the CFS, the Profile Commission, the Eurasian Association of Clinical Pharmacology and Pharmacovigilance, and the Association of Clinical Pharmacologists.
Deadline: December 1, 2026 (for the time‑motion study); April 1, 2027 (for tariff rates).
5. Organise interaction with professional associations and the Central Expert Council on Quality Control of Medical Care (TsEKKMP) of the Ministry of Health of Russia to include clinical pharmacologists in working groups for the development of clinical guidelines (especially in the fields of cardiology, oncology, gastroenterology, neurology, psychiatry, pulmonology, rheumatology, haematology, anaesthesiology and intensive care, paediatrics, geriatrics, surgery, obstetrics and gynaecology, and other areas) with the aim of "embedding" the consultation of a clinical pharmacologist and expert review of pharmacotherapy sections, including:
incorporating clinical‑pharmacological technologies of personalised medicine (pharmacogenetic testing, therapeutic drug monitoring);
including information on drug‑drug interactions;
including tools for combating polypharmacy / deprescribing algorithms;
harmonising the information presented in clinical guidelines in accordance with regulatory documents on the circulation of medicinal products, including the summaries of product characteristics.

6. Explore the feasibility of establishing and propose a concept for a National Research Medical Centre for Clinical Pharmacology based on a scientific or educational organisation, including a Drug Information Telemedicine Centre for organisational, methodological, medical‑consultative and scientific work in the field of clinical pharmacology.
Executors: jointly the CFS, the Profile Commission, the Eurasian Association of Clinical Pharmacology and Pharmacovigilance, and the Association of Clinical Pharmacologists.
Deadline: February 1, 2027.
7. Form a working group and organise training for clinical pharmacologists to conduct expert reviews at various levels (medical organisation, regional, federal) on issues of interchangeability and pharmacotherapeutic substitutions, as well as pharmacovigilance.
Executors: jointly the CFS, the Profile Commission, the Eurasian Association of Clinical Pharmacology and Pharmacovigilance, and the Association of Clinical Pharmacologists.
Deadline: October 1, 2026.
8. Develop recommendations on the organisation and procedure for the work of formulary committees in medical organisations as a tool for clinical pharmacologists to determine drug policy in a medical organisation. Harmonise the drug formularies of medical organisations with current clinical guidelines and the summary of product characteristics for medical use.
Executors: jointly the CFS, the Profile Commission, the Eurasian Association of Clinical Pharmacology and Pharmacovigilance, and the Association of Clinical Pharmacologists.
Deadline: February 1, 2027.
9. Apply to the Department of State Regulation of Medicines Circulation of the Ministry of Health of Russia with a proposal to consider clinical pharmacologists as principal investigators in clinical trials, provided that the research team includes physicians of other specialties, in view of the integrative nature of clinical pharmacology and the competencies of clinical pharmacologists to structure and control the work of a research centre.

Deadline: October 1, 2026.
10. Develop and approve recommendations of professional associations on clinical pharmacology for teaching the discipline "Clinical Pharmacology" at the undergraduate and residency levels, recommending to universities: basic volumes of the discipline "Clinical Pharmacology"; mandatory sections (modules) of the discipline for all specialties; requirements for practical training of students; forms of current and interim assessment. The recommendations must not contradict the federal state educational standards.
Executors: jointly the Educational and Methodological Commission, the Eurasian Association of Clinical Pharmacology and Pharmacovigilance (within the framework of the Education Committee), and the Association of Clinical Pharmacologists.
Deadline: December 1, 2026.
11. Develop recommendations of professional associations on clinical pharmacology for career guidance work regarding the specialty "Clinical Pharmacology", teaching the discipline "Clinical Pharmacology" at the residency level, as well as mentoring of young clinical pharmacologists (within the framework of the Education Committee of the Eurasian Association of Clinical Pharmacology and Pharmacovigilance).
Executors: jointly the Educational and Methodological Commission, the Eurasian Association of Clinical Pharmacology and Pharmacovigilance (within the framework of the Education Committee), and the Association of Clinical Pharmacologists.
Deadline: November 1, 2026, and thereafter on an ongoing basis.
12. Organise the work of regional Chief Freelance Specialists in clinical pharmacology to establish target admission figures for residency and applications for professional retraining cycles in clinical pharmacology, as well as to coordinate the formation of applications for advanced training cycles for clinical pharmacologists and to select candidates for training under the state assignment.
Executors: jointly the CFS, the Profile Commission, the Eurasian Association of Clinical Pharmacology and Pharmacovigilance, and the Association of Clinical Pharmacologists.
Deadline: September 1, 2026.
13. Continue work on updating the assessment tools funds for primary and primary specialised accreditation, as well as for awarding qualification categories in clinical pharmacology.
Executors: the CFS and the Educational and Methodological Commission.

14. Take the initiative and organise the development of network programmes of continuing professional education for clinical pharmacologists and related specialties:
"Rational Antimicrobial Therapy and Prevention" (72 hours);
"Clinical Pharmacogenetics with Basics of Personalised Pharmacotherapy" (72 hours);
"Antithrombotic Therapy and Prevention" (72 hours);
"Polypharmacy in a Medical Organisation: Problems and Solutions" (72 hours);
"Pharmacovigilance" (72 hours);
"Methodology of Clinical Trials. Real‑World Clinical Practice Studies" (72 hours).
Executors: jointly the CFS, the Educational and Methodological Commission, the Eurasian Association of Clinical Pharmacology and Pharmacovigilance, and the Association of Clinical Pharmacologists.
Deadline: March 1, 2027.
15. Take the initiative to allocate a state assignment for advanced training of teachers in clinical pharmacology at all levels (undergraduate, residency, continuing professional education) to the leading departments of clinical pharmacology.

16. Propose to the Ministry of Health of Russia to amend Order of the Ministry of Health of Russia dated July 25, 2001 No. 703 "On Approval of the Regulations on the Competition 'Best Doctor of the Year'", supplementing the list of nominations with the category "Best Clinical Pharmacologist".
Executors: jointly the CFS, the Profile Commission, the Educational and Methodological Commission, the Eurasian Association of Clinical Pharmacology and Pharmacovigilance, and the Association of Clinical Pharmacologists.
Deadline: September 1, 2026.
17. Develop a concept and hold annually:
an Olympiad in Clinical Pharmacology for students and residents;
"Thematic Battles" in Clinical Pharmacology for clinical pharmacologists.
Executors: the CFS, the Profile Commission, the Educational and Methodological Commission, the Eurasian Association of Clinical Pharmacology and Pharmacovigilance, and the Association of Clinical Pharmacologists.
Deadline: March 1, 2027.
18. Compile a register of regional Chief Freelance Specialists in clinical pharmacology and clinical pharmacologists of medical organisations.
Executors: the CFS, the CFS of Federal Districts, and the Profile Commission.
Deadline: September 1, 2026.
19. Form a unified annual plan of scientific‑practical and corporate events for clinical pharmacologists. For 2027, draw up a plan of scientific‑practical events on clinical pharmacology, as well as cultural and sporting events for clinical pharmacologists, dedicated to the 30th anniversary of the medical specialty "Clinical Pharmacology".
